In the final episodes, *Hyper Knife* goes full throttle as Se-ok and Deok-hee face their ultimate challenge — outsmarting the law, handling betrayals, and confronting their twisted bond. Things kick off with Se-ok being lured into a police trap by a broker, but Deok-hee intervenes and kills the broker. Young-joo helps cover it up, though he’s horrified. Se-ok is oddly impressed and agrees to perform a high-risk surgery on Deok-hee.

Their dark mentor-mentee dynamic deepens as they reconcile at a funeral. Deok-hee reveals he ruined Se-ok’s life years ago to stop her from being used by a greedy surgeon, Myung-jin — a twisted act of protection. Se-ok finds this funny and admits she would’ve killed Myung-jin too, cementing how similar they really are.

Meanwhile, Inspector Yang, Deok-hee’s old frenemy, is closing in. To protect Se-ok, Deok-hee frames Yang by exposing Detective Lee’s corpse and revealing a damning voice recording implicating Yang in corruption. When Yang pushes Se-ok to let Deok-hee die to bury the evidence, she instead threatens him and ends up killing him with Young-joo’s help after a brutal confrontation.

As Deok-hee deteriorates, we learn he’s been intentionally sabotaging his health — taking banned drugs and worsening his condition — to7 create a surgical challenge that will force Se-ok to grow. His goal: die on her table and make her a better surgeon. Se-ok, however, is determined to save him.

In the end, Deok-hee agrees to the surgery, and just before it starts, he reaches out to shake Se-ok’s hand, believing he won’t survive. She refuses and confidently preps for surgery, and then — the screen cuts to black.

In a bonus scene, Se-ok, alive and seemingly thriving, is seen preparing for black market surgery, waiting for someone to join her. We don’t see their face, just their feet — but it’s heavily implied to be Deok-hee.

The show ends ambiguously but powerfully, with a dark, weirdly heartwarming tone between two brilliant, unhinged surgeons now possibly working side by side. Murder may no longer be their main outlet — now, it’s brain surgery.,
